Descripción Introduction: Pulmonary hypertension assessed by echocardiogram in patients with COVID-19 has been poorly studied, and its impact on clinical outcomes remains uncertain. Objective: To evaluate the relationship between the echocardiographic probability of pulmonary hypertension and morbidity and mortality in patients with COVID-19, using the ROX index as a predictor. Materials and Methods: A retrospective study was conducted involving 306 patients with COVID-19 confirmed by antigen or molecular tests. Statistical analyses included Chi-square, Fisher's exact test, Student's t-test, and Mann-Whitney test. The odds ratio (OR) for the ROX index was adjusted for age and gender, with confidence intervals exceeding 95%. Results: Survivors exhibited higher ROX values at 2 hours (5.8) and 12 hours (7.8) compared to those who died (4.5 and 4.9, respectively). The OR for ROX was 8.5 at 2 hours and 17.6 at 12 hours. A significant correlation was found between a higher probability of pulmonary hypertension and lower ROX values (p = 0.048), as well as with increased mortality (p = 0.037). Patients over 70 years of age with more comorbidities showed lower ROX values and higher mortality rates. Discussion: These findings suggest that pulmonary hypertension may serve as a relevant marker of severity in patients with COVID-19, correlating with worse outcomes. Conclusions: It is recommended to increase the sample size in future studies to validate these results and enhance interventions in this population.
Descripción Introducción: La hipertensión pulmonar evaluada mediante ecocardiograma en pacientes con COVID-19 ha sido poco estudiada, y su impacto en los desenlaces clínicos es incierto.Objetivo: Evaluar la relación entre la probabilidad ecocardiográfica de hipertensión pulmonar y la morbimortalidad en pacientes con COVID-19, utilizando el índice ROX como predictor.Materiales y Métodos: Se realizó un estudio retrospectivo de 306 pacientes con COVID-19 confirmados por pruebas antigénicas o moleculares. Se llevaron a cabo análisis estadísticos que incluyeron Chi-cuadrado, prueba exacta de Fisher, T de Student y Mann-Whitney. El odds ratio (OR) del índice ROX se ajustó por edad y género, presentando intervalos de confianza superiores al 95%. Resultados: Los supervivientes mostraron valores más altos de ROX a las 2 horas (5,8) y a las 12 horas (7,8) en comparación con los fallecidos (4,5 y 4,9, respectivamente). El OR del ROX fue 8,5 a las 2 horas y 17,6 a las 12 horas. Se encontró una correlación significativa entre una mayor probabilidad de hipertensión pulmonar y valores bajos del ROX (p = 0,048), así como con mayor mortalidad (p = 0,037). Los pacientes mayores de 70 años, con más comorbilidades, mostraron menor ROX y mayor mortalidad.Discusión: Estos hallazgos sugieren que la hipertensión pulmonar podría ser un marcador relevante de gravedad en pacientes con COVID-19, asociándose a peores desenlaces.Conclusiones: Se recomienda ampliar el tamaño de la muestra en futuros estudios para validar estos resultados y mejorar las intervenciones en esta población.
