Chest X-rays (CXR) are a fundamental tool in the diagnosis of respiratory diseases. This study evaluated their effectiveness with the aim of optimizing the clinical management of patients. A retrospective study of clinical cases in primary care centers (PHC) in Panama was conducted, analyzing 150 clinical records of patients over 18 years of age with suspected acute respiratory infection (ARI) who were treated between January 2022 and December 2024. The inclusion criteria covered adults with acute symptoms, formal RxTx request, official radiological report, and complete file. Data confidentiality was guaranteed in accordance with regulations, and the analysis was performed using SPSS version 26 and the chi-square test for associations (p < 0.05). The results suggest a high concordance between radiological findings and clinical diagnosis, demonstrating a significant improvement in therapeutic accuracy. However, deficiencies were identified in the formulation of medical requests. The systematic integration of RxTx into PHC protocols, together with standardized clinical guidelines and continuous training, is proposed as a strategy to optimize the rational use of resources, reduce diagnostic error, and improve the clinical outcomes of IR, reinforcing the role of radiology in family and community medicine.

La radiografía de tórax (RxTx) es una herramienta fundamental en el diagnóstico de enfermedades respiratorias. Este estudio evaluó su efectividad con el propósito de optimizar el manejo clínico de pacientes. Se realizó un estudio retrospectivo de casos clínicos en centros de atención primaria (APS) en Panamá, analizando 150 expedientes clínicos de pacientes mayores de 18 años con sospecha de infección respiratoria aguda (IR), atendidos entre enero de 2022 y diciembre de 2024. Los criterios de inclusión abarcaron adultos con sintomatología aguda, solicitud de RxTx formal, informe radiológico oficial y expediente completo. La confidencialidad de los datos se garantizó según normativas, y el análisis se efectuó con SPSS versión 26 y la prueba de chi-cuadrado para asociaciones (p < 0.05). Los resultados sugieren una alta concordancia entre los hallazgos radiológicos y el diagnóstico clínico, lo que demuestra una mejora significativa en la precisión terapéutica. No obstante, se identificaron deficiencias en la formulación de las solicitudes médicas. Se propone la integración sistemática de la RxTx en protocolos de APS, junto con guías clínicas estandarizadas y capacitación continua, como estrategia para optimizar el uso racional de recursos, reducir el error diagnóstico y mejorar los desenlaces clínicos de las IR, reforzando el rol de la radiología en la medicina familiar y comunitaria.
