Descripción Making mistakes is an inevitable part of life for every human being, and it has been described that the perspectives for addressing them are heterogeneous. However, the healthcare personnel involved do not always have the resources to manage the inherent risks within healthcare systems, where the approach must be collective, and attempts with a much more preventive vision play a more important role than punishment and blame. Systematized strategies such as James Reason's Swiss Cheese Model allow for the management of unsafe aspects, as well as the establishment of timely interventions that minimize the occurrence of errors.The objective of the review is to explore the different schools of thought related to the blame-centered punitive culture, as well as to propose the usefulness of James Reason's model, which contributes to generating interventions in the identified causal chains to reduce the risk of errors and/or accidents. Method: A review of published information, as well as expert interpretations of interview and open access digital books from the Pub Med, Elsevier, Google Scholar, Scielo, Medigraphic, and Academia databases in Spanish, English, French, and Portuguese was conducted. Keywords (Patient Safety, Medical Errors, Potential Adverse Event, Guilt, Risk) were used, and publication dates fell between 2000 and 2024. Conclusions: The complexity of healthcare settings, as well as the multi-causal interaction, favors the emergence of errors; however, the co-creation of spaces that encourage feedback and analysis rather than seeking blame, will be essential to overcome obstacles and improve existing processes.
Descripción Introducción: Cometer errores es parte inevitable de la vida de todo ser humano y está descrito que las perspectivas de abordaje son heterogéneas. Sin embargo, el personal sanitario involucrado no siempre dispone de recursos para gestionar los riesgos inherentes dentro de los sistemas sanitarios, en donde el enfoque debe ser colectivo y los intentos con una visión mucho más preventiva, las cuales juegan un papel más preponderante que el castigo y la culpabilización. Estrategias sistematizadas como el Modelo del Queso Suizo de James Reason permiten gestión de aquellos aspectos inseguros, así como el establecimiento de intervenciones oportunas que minimizan la aparición de los errores. El objetivo de la revisión es explorar las distintas corrientes de pensamiento relacionadas a la cultura punitiva centrada en la culpabilidad, así como proponer la utilidad del modelo de James Reason el cual contribuye a generan intervenciones en las cadenas causales identificadas con el fin de reducir el riesgo de errores y/o accidentes. Método: se realiza una revisión de información publicada, así como de interpretaciones de expertos en entrevistas y libros digitales con acceso abierto de base de datos Pub Med, Elsevier, Google Académico, Scielo, Medigraphic, Academia en idiomas español, inglés, francés y portugués. Por medio de la utilización de palabras claves (Seguridad del paciente, Errores Médicos, Potencial Evento Adverso, Culpa, Riesgo) y cuya fecha de publicación estuviese dentro del periodo 2000 y 2024. Conclusiones: La complejidad de los entornos sanitarios, así como la interacción multicausal favorece la aparición de errores, sin embargo, la co-creación de espacios donde se favorezca la retroalimentación y el análisis en lugar de buscar culpables serán fundamentales para superar obstáculos y mejorar los procesos existentes.
